The church is attested in the 10th century, but the current building probably dates back to 1056, while the official consecration took place only in 1148. It is certain that it was renovated in 1239 and the staircase was added in 1263.

Again in 1460 the church was enlarged, based on a project by Giuliano da Maiano, who lengthened the cross and the presbytery and designed the new chapels of Santa Fina and della Concezione.

The facade that dominates the square is from the Romanesque period and dates back to the thirteenth-century arrangement: simply bare, it has two portals on the sides and three circular windows.

The highest part of the counter-façade is decorated with a Last Judgment by Taddeo di Bartolo, which continues on the adjacent walls of the left aisle with Heaven and on the right with Hell.

The four Prophets and the Cardinal Virtues, made by the same author, are depicted in the arches. The four Evangelists in the vault of the first span are also Taddeo di Bartolo.

Below the Last Judgment, there is the Martyrdom of San Sebastiano by Benozzo Gozzoli. The paintings on the side pillars are also by the same artist: on the left an Assunta and San Bernardo and Sant'Agostino, on the right a Sant'Antonio Abate and San Girolamo and San Bernardino.

On the two shelves on the sides, in front of fake damask carpets, there are the precious wooden statues of the Archangel Gabriel and of the Madonna Annunciata, which form the group of the Annunciation by Jacopo della Quercia and in the upper register, there is an Eternal Father surrounded by Angels .

In the area of the right aisle, around the oculus, there is a fresco with two peacocks, animals with ancient Christian symbolism as it was believed that their flesh never rotted, like the word of Christ. Furthermore, on the inside of the pillar, there is a Santa Fina by Lippo Memmi.
